Aquesta ruta permet conèixer un dels millors miradors sobre el Camp de Tarragona i el cercle de muntanyes que l'envolten, des del Montsant i les muntanyes de Prades fins als cims de Vandellòs. Parteix del poble de Riudecols i, per una forta pujada, arriba al santuari situat en un punt estratègic. La baixada fins Alforja es fa des del coll de Cortiella. En total són uns 2xxx km, amb un desnivell moderat.El més atractiu d'aquesta ruta són les vistes que tenim en tot moment (sobretot des del santuari) i els boscos ben conservats de què podrem fruir. La Mare de Déu de Puigcerver té molta devoció a Alforja. Hi han referències de l'ermita de 1227. Segons la llegenda que es conta a la comarca, els habitants d'Alforja veien de nit unes llengües de foc que baixaven del cel en el lloc on actualment hi ha l'edifici. Com tantes altres llegendes relacionades amb els cultes marians, quan la gent anà a veure què hi passava hi trobaren una imatge de la Mare de Déu, la qual intentaren fer baixar al poble, i per tres vegades retornava al lloc on l'havien trobada. A conseqüència d'això s'aixecà l'ermita en aquest indret, i l'edifici s'engrandí i reformà als segles XVII i XIX.Segons el folklorista Joan Amades es tracta de la cristianització, amb l’advocació de la verge, d’un antic lloc de culte pagà, hipòtesi que confirmaria la tradició d’encendre imponents foguerades la nit de Sant Joan, davant de l’ermita, fins al final del segle XIX.Aquest indret forma part de la unitat de paisatge Escornalbou-Puigcerver. Gràcies al seu relatiu aïllament, la seva alçada i les seves característiques físiques i climàtiques, acull una zona boscosa d'alt interès, sobretot per la presència d'exemplars arboris de grèvol, poc freqüent en aquesta part de Catalunya. L'alzinar es troba reclòs a les parts més altes. Per sobre dels 700 m d’altitud l’alzinar litoral deixa pas a l’alzinar muntanyenc, més ric en caducifolis com la blada (Acer opalus) i la moixera (Sorbus aria). També trobem pinedes de pi blanc, brolles d'estepes i brucs i algunes clapes de pinedes de pinassa i de roure de fulla petita.Puigcerver és, juntament amb el tossal i el castell d’Escornalbou, la Mare de Déu de la Roca a la muntanya de l’Areny i l’embassament de Riudecanyes, un dels elements de referència de la plana del Baix Camp.un dels elements de referència de la plana del Baix Camp.
